And, here's a nice little gem. When I came on shift today, the patient with VRE's chart was missing. The residents had taken it on rounds. And weren't anywhere to be found. At this point, neither the nurse coming off shift, my preceptor or myself knew that the patient had free case law tested positive for VRE. The doctors had noted it in the chart, but took the chart with them. And though the patient was supposed to be on contact precautions because of the VRE, they hadn't had anyone put up a sign alerting people to the precautions which is standard practice or even uttered a word about it to anyone. |
